How to get a Philhealth Number
You may be a fresh graduate, someone who recently resigned from a job, or perhaps been working for a long time and alas! you found out you don't have a philhealth number when you tried to use it during a hospitalization...Well, here's what you need to do to get a number:
For New Enrollees:
For those who would like to add their dependents:
Legal dependents could be the following:
You may visit any Philhealth Office in Cebu. There are currently four offices in the province namely Danao, Mandaue, Carcar and Cebu City

For New Enrollees:
- Get a PMRF (Philhealth Membership Registration Form) and fill it up
- Attach a photocopy of your birth certificate or if you don't have a copy, attach two valid ID's (SSS, TIN, Driver's license, Voters ID)
- Pay the minimum premium of Php 300.00/quarter ( premiums will increase in July 2012) - for voluntary members only
- For employed members -get an ER2- Philhealth employer form and have your HR personnel or manager signed it before going to any philhealth office together with the docs above except #3.
For those who would like to add their dependents:
Legal dependents could be the following:
- Legal spouse who is not employed or a member of Philhealth
- Children below 21 years old
- Parents who are 60 years old and above; didn't acquire any medicare credits in the past
